Completing a C-414

Requesting Agency, Court and Offence Information

1.

  • Always include your file number for the occurrence. In the case of a subsequent submission, also include the laboratory file number (when known).

  • Complete all fields related to your agency and contact information. Double-check details for accuracy.

  • When known, ensure the trial date is indicated.

  • Always include the most serious Criminal Code offence information related to the occurrence.

  • Note: Answering the question "Have any exhibits in this file been examined or will be examined by any other laboratory?" is required prior to approval. 

Occurrence Summary

  • Provide a concise account of the occurrence keeping to details relevant to the laboratory examination.

  • Include all required information specific to the requested examinations or exhibits.

  • Indicate why the requested exhibits / examinations are significant.

  • Indicate how the results will assist with the investigation.

Subject Information

  • Include subject information relevant to the occurrence. 

  • In particular, an entry should be made for any individual from whom an exhibit listed on the C-414was seized.

List of Exhibits and Services Requested

  • Exhibit descriptions and unique exhibit identifiers must be indicated on the C-414. (Note: Exhibit identifiers are restricted to 14 characters on the C-414.)

  • Exhibit identifiers on the C-414 must be identical to those marked on the package. Refer to important information about exhibit identifiers and package markings.

  • List only one (1) exhibit per field on the C-414. Multiple services can be selected for a single exhibit.

Exhibit Details

  • For multiple items in a single exhibit package, provide a detailed list of all contents in the description (or synopsis if space is limited).

  • If the exhibit is a swab, describe what was swabbed. (e.g. swab of possible blood from bedroom wall, global swab from knife handle, vaginal swab, etc.)

  • Indicate from who or where the exhibit was originally seized - not whom you believe the items or DNA belongs to.

Final Steps

  • Double-check that all required information and forms are included in your request.

  • Submit via email to FAC-CEJ@rcmp-grc.gc.ca

  • Ensure the appropriate transmission guidelines are followed for the security level of the file.

Specific information is often required.

Requests received without all required information are rejected.
