Form FAI E01 - Firearms & Toolmark Identification Check Sheet

We are writing to inform you that effective June 28, 2023, we will require a completed firearms check sheet for each forensic firearms examination requested. This requirement is necessary to ensure that the submission is “laboratory ready” as possible to avoid processing delays.
